Currently, I am in network with Anthem, Blue Cross, Blue Shield, and  United Health Care insurance and also accept private-pay clients and out-of-network insurances.

 

If I am out-of-network with your insurance, I can provide you with a receipt or “superbill” to help you receive reimbursement from your insurance company.

 

Please be sure to contact your insurance provider so you are familiar with your insurance coverage, what your copay is, and what benefits you have for out-of-network treatment.

I accept cash, checks, and credit card.  Payment is expected at time of service unless other arrangements are made in advance.

     ● Follow-up Nutrition Assessment (1 hour):  $120
